No senior wants to live in a nursing home if they have another option. H.O.M.E.’s newest program helps capable seniors leave nursing homes by connecting them with the support services for independent living in the community.

H.O.M.E., along with Access Living and Catholic Charities, has collaborated with the Illinois Department on Aging to help transition seniors back to the community. The Home Again program identifies and transitions seniors living in nursing homes into apartments of intergenerational housing. The program provides furniture, necessary household items, and clothing for free. If the senior is moved into an apartment he receives one month of food, the security deposit, and one month’s rent for free.
